Bug - Fresh Installation - Version 2.9 - All Menu Items return same error

A fix will be introduced for this issue in version 3.0. In the meantime, you can use replace the following file (modules\reports\views\prepare_add_item.php

with the attached (prepare_add_item.php) from Version 2.8. The replacement provides a temporary fix for the issue.
