Automate action - add record to Sub-entity duplication

I have a series of tasks that must be added for each project stage. I created one Automated Process for each Stage and added several actions of the type "Sub-entity Tasks - add record".
All the tasks are created, but one of the variables ("Name") gets repeated, while the other variables are correctly displayed and recorded in the database.
"Name" is a Heading, simple input text field, and required. I am not sure what's wrong. Please see the screenshot.
