I was creating a new entity and I copied three similar fields from another entity. The fields created in this way are appearing in entity field list, but not appearing in form configuration. See images below.
(I did check/repair database and clear cache/cookies).

Found this issue. File from archive replace to modules\entities\actions\
Remove fields that you copied and copy them again.
